1. Motor Insurance Claims (Accident, Windscreen & Theft)
- •Completed Underwriter Motor Claim Form (signed by the policyholder)
- •Police Abstract (Original) reporting the accident or theft within 24–48 hours
- •Copy of the Driver’s valid Driving License and National ID
- •Copy of the Vehicle Logbook and ETR Receipt (for Windscreen replacement claims)
2. Health & Medical Claims (Reimbursement & Overseas Treatment)
- •Completed Medical Claim Form signed by both the patient and the attending doctor
- •Original itemized hospital invoices, ETR payment receipts, and prescription notes
- •Diagnostic laboratory/imaging requests and Case Summary / Discharge Summary (for In-Patient)
3. WIBA, Domestic Package, Business & Marine Cargo Claims
- •WIBA / Personal Accident: DOSH Form 1, Medical Report, Sick-off sheets, Payslips & Employee ID
- •Home / Fire / Burglary: Police Abstract, purchase invoices/receipts of damaged/stolen items, and repair quotations
